List of Acceptable IDs for Philippine Passport Application

Applicant must present an original with a photocopy of the following valid identification:
  • Philippine Identification (PhilID)
  • Electronic Philippine ID
  • Digital National ID (*Applicants must submit a printed copy of the digital ID and will be required to generate the digital ID via the website: https://national-id.gov.ph/ or the eGovPH mobile application during processing for document verification.

The following IDs may also be used in lieu of the Philippine Identification (PhilID) / ePhilID:

  • Social Security System (SSS) Card
  • Government Service Insurance System (GSIS) Card
  • Unified Multi-Purpose Identification (UMID) Card
  • Land Transportation Office (LTO) Driver’s License. Student Permit may be accepted if in card format.
  • Bangsamoro Land Transportation Office (BLTO) - issued Driver's License (DL) Cards issued 2022 onwards
  • Professional Regulatory Commission (PRC) ID
  • Overseas Workers Welfare Administration (OWWA) E-Card
  • Commission on Elections (COMELEC) Voter's ID or Voter's Certificate issued from COMELEC main office in Intramuros, Manila.
  • Philippine National Police (PNP) Permit to Carry Firearms Outside Residence
  • Senior Citizen ID
  • Airman License (issued August 2016 onwards)
  • Philippine Postal ID
  • Seafarer's Record Book (SRB) or Seafarers Identity Document (SID) (*must be issued Feb 2020 onwards) issued by the Maritime Industry Authority (MARINA)
  • Valid or Latest Passport (For Renewal of Passport)
  • School ID (if applicable)
    • For minor applicants (if school ID is not applicable) Certificate of Enrolment with photo of minor and dry seal of school
    • For adult applicants School ID and Certificate of Registration
Note
  • For applicants based overseas, they may use their host government issued IDs showing their Philippine citizenship. (Example: Residence Card)
  • All IDs presented should be readable, untampered and contain consistent information with the documents presented upon application.

Important Reminder: The Department may require additional supporting documents as may be necessary.

Please be advised that proof of emergency or urgent travel (medical certificate, valid working visa or permanent residence card for OFWs returning overseas, enrolment confirmation or acceptance letter from schooling institution, et al) must be presented for evaluation.

Passport Certification Process and Requirements

  1. Download and fill up the Passport Certificate Request Form

  2. Email This email address is being protected from spambots. You need JavaScript enabled to view it. and indicate the following on the subject line: Passport Certificate Request. The following should be included in the email:
    1. Filled up and signed Passport Certificate Request Form
    2. Clear scanned copy of current passport data page (for requests for Certified True Copy of Passport or Certificate of Passport Issuance)
    3. Number of certificates needed
    4. For parents/legal guardian requesting a Certificate of No Passport Issuance for a child (aged 17 years and below), scanned copy of child’s Birth Certificate AND Passport or ID of requesting parent/legal guardian (present Letter of Guardianship issued by Family Court)
  3. A Passport Division representative will confirm receipt of your email and schedule your visit for collection of the certificate at DFA ASEANA or other Consular Offices. A Php 100 fee shall apply and shall be made at the Consular Office or Foreign Service Post. For requests made at Foreign Service Posts, a USD 25 fee or its local currency equivalent shall apply.

  4. If the certification is being claimed by an immediate family member, the passport holder should execute a written authorization letter and provide a copy of his/her passport/valid government ID. Claimant shall also be required to present his/her passport or a valid government ID. If the person claiming is not an immediate family member, a notarized SPA with ID of the passport holder and ID of the claimant is required.

  5. In view of the current COVID-19 threat, requesting party is strongly advised to avail of courier service delivery/provide a self-addressed, stamped and pre-registered postal envelope (where applicable).

Notes:

  • Certificates may be requested from the Office of Consular Affairs at DFA ASEANA or at any Consular Office or Foreign Service Post.
  • Certificates are issued after 5 working days

CORE REQUIREMENTS FOR PASSPORT RELEASING

  • Original DFA issued receipt
  • Original acceptable ID
  • Old Passport (for renewal applications)

In case of a lost receipt, the applicant or authorized representative is required to present the Original notarized Affidavit of Lost Receipt executed by the applicant


Passports unclaimed after one (1) year will be cancelled per Department Order No. 2021-012.

In case the applicant is unable to claim his/her passport, immediate family members* are allowed to claim the passport in behalf of the applicant

  • Authorization letter (must mention affiliation with applicant),
  • Original DFA issued receipt,
  • Valid ID (original and photocopy) of applicant and representative
    *Immediate family members of legal age such as parents, brother, sister, spouse or children

In case a person other than an immediate family member will claim the applicant's passport

  • Original notarized Special Power of Attorney (SPA) executed by the applicant, issued to the authorized representative
  • Valid ID (original and photocopy) of applicant and representative indicated in the Special Power of Attorney

For minor applicants

  • If passport will be claimed by mother, claimant must present original DFA issued receipt and valid ID
  • If passport will be claimed by father, claimant must present original DFA issued receipt, valid ID and PSA documents indicating proof of parental authority over the minor
  • If minor is illegitimate and passport will be claimed by a person other than the mother, claimant must present original DFA issued receipt, Special Power of Attorney (SPA) executed by the mother and valid ID of the claimant indicated in the Special Power of Attorney
