Please be informed that DFA with authentication services are accepting applicants through the following options:
OPTION 1: WALK-IN (Limited slots per day depending on the capacity of the Consular Office https://dfa-oca.ph/authentication/authentication-general-info/) You may avail the walk-in facility if you are:
  1. Document owners (If the documents are under their name); or
  2. Documents of immediate family members (If the documents are under the name of their family relatives).
To process, the authorized representative has to bring the following:
  1. Proof of kinship must be presented such as birth certificate, marriage certificate, IDs, among others;
  2. Signed authorization letter (SPA for minor document owners);
  3. Copy valid government ID bearing document owner's signature;
  4. Representative's valid ID (photocopy and original).
OPTION 2: ONLINE APPOINTMENT (through website https://www.apostilleonline.ph) You may avail the online appointment facility if you are:
  1. Document owners or document of immediate family members; or
  2. Authorized Representatives that are not family related to the document owner (i.e. liaison officers).
To process, the authorized representative has to bring the following:
  1. Signed authorization letter (SPA for minor document owners);
  2. Copy valid government ID bearing document owner's signature;
  3. Representative's valid ID (photocopy and original).

ADVISORY: Please be informed that the DFA’s Apostille Application & Appointment System (AAAS) is still under develop to become available to all Consular Offices. Walk-in transactions services will continue to be accepted. The Department requests the public’s continued understanding on this matter.
  Procedures for Application
WALK-IN
  1. Fill-up application form given and present at the Verification Window Counter.
  2. Prepare the necessary documents before you proceed to the processing window.
  3. Present your valid government-issued ID (original and photocopy) together with the documents to the processor for assessment.
  4. Pay corresponding fees to the Cashier.
ONLINE APPOINTMENT
  1. PRINT and present your confirmed appointment letter at the Appointment Verification Window Counter.  (EDITING, REVISING OR ALTERING THE APPOINTMENT LETTER IS CONSIDERED FALSIFICATION OF PUBLIC DOCUMENT/S AND IS PUNISHABLE BY LAW).
  2. Prepare the necessary documents before you proceed to the processing window.
  3. Present your valid government-issued ID (original and photocopy) together with the documents to the processor for assessment.
  4. Pay corresponding fees to the Cashier.
  FILING/Claiming of Apostille Documents
IF DOCUMENT OWNER
  1. Official Receipt; and
  2. Original valid government issued ID.
IF AUTHORIZED REPRESENTATIVE
  1. Official Receipt;
  2. Authorization letter from owner (for minor applicants original Special Power of Attorney executed by Parent/s);
  3. Photocopy of the owner’s valid government-issued ID bearing his/her signature; and
  4. Photocopy and original copy of representative’s valid government issued ID.
  List of Acceptable IDS:
   ALL DIGITIZED PHILIPPINE GOVERNMENT-ISSUED IDs BEARING THE SIGNATURE OF THE APPLICANT (SSS, UMID, Driver’s License, PRC, Postal ID, PhilSys ID, etc.)

Reminder: All IDs presented should be readable, untampered, and contains consistent information with the documents presented upon application.
IMPORTANT NOTICE:

All unclaimed document/s will be disposed of after three (3) months from the scheduled date of release.

Processed documents subject for further verification will only be authenticated upon confirmation of the authenticity of the document by the issuing office.

Processed documents that are not in order (pending documents) will only be authenticated upon compliance of the Authentication requirements.

GENERAL INFORMATION

“Authentication” is a generic term that commonly refers to the process of verifying the origin of a public document by certifying the authenticity of the signature and legal authority of the public official who signed it. It does not, however, certify the content of the public document to which it relates.


On 14 May 2019, the Hague Convention Abolishing the Requirement of Legalization for Foreign Public Documents (Apostille Convention) entered into force for the Philippines.


Thenceforth, an Apostille is attached, as proof of authentication, to the underlying Philippine public document that will be used abroad. After authentication by the DFA, there is no more need for legalization by the concerned Foreign Embassy / Consulate if the country of destination of the authenticated document is also a State Party to the Apostille Convention (except for Finland, Germany and Greece as they have objected to the Philippines’ accession).


However, if the country of destination of the document is NOT a State Party to the Convention, or if the document will be submitted in Finland, Germany, or Greece, legalization by the concerned Foreign Embassy / Consulate is still required.

The following types of documents may also be authenticated in DFA San Fernando, Pampanga (2F Robinsons Star Mills, City of San Fernando, Pampanga):
 
 
1. Birth/Marriage/Death Certificate/Certificate of No Marriage Record and/or Negative Records (CENOMAR)
• Certificates must be on Security Paper issued by the Philippine Statistics Authority (PSA), formerly National Statistics Office (NSO). If the document is registered at the Local Civil Registrar (LCR) within six (6) months from date of registration, it must be certified/authenticated by PSA.

Note: LCR copy of Marriage Certificate, Birth Certificate or Death Certificate may be required in cases when entries from PSA/NSO are unreadable.

2. Transcript of Records (TOR) and Diploma (For State Colleges and Universities)
• Certified True Copies from the school, and
• Certification, Authentication and Verification (CAV) from the school and signed by the School/University Registrar and/or school’s authorized signatory.

3. Transcript of Records (TOR) and Diploma (Private Schools and Local Universities and Colleges)*
• Certified True Copies from the school, and
• Certification, Authentication and Verification (CAV) from the Commission on Higher Education (CHED) where the school is located.

4. Transcript of Records (TOR) and Diploma / National Certificate (Technical or Vocational Courses)*
• Certified True Copies from the school, and
• Certification, Authentication and Verification (CAV) from Technical Education and Skills Development Authority (TESDA) where the school is located.

5. Form 137 and Diploma (High School and Elementary Level)*
• Certified True Copies from the school, and
• Certification, Authentication and Verification (CAV) from Dep-Ed Regional Office where the school is located.

6. Medical/AIDS Free Certificate*
• Certified/Authenticated by the Department of Health (DOH)

Note: Applicable only for use to the following countries which requested the said authentication: Spain, Palau, Libya, Oman, Cuba, Portugal, Greece, Cyprus and Angola). If not included, submit the same requirements stated in #10.

7. Civil Aviation Authority of the Philippines (CAAP) issued licenses*
• Certified/Authenticated by CAAP

8. Driver’s Licenses*
• Certification from Land Transportation Office (LTO Main Branch only)

9. Professional Licenses/Board Certificates/Board Ratings/Certifications*
• Certified/Authenticated by Professional Regulations Commission (PRC)

10. Certificate of Employment/Trainings/Seminars, Baptismal Certificate and other documents issued by a private entity
• Notarized affidavit, stating necessary factual circumstances and indicating certificate/s as attachment/s
• Certificate of Authority for a Notarial Act (CANA) signed by the Executive Judge or Vice Executive Judge from the Regional Trial Court which issued the commission of the Notary Public

Note: Copy of Notarial Commission is not the same as Certificate of Authority for a Notarial Act (CANA).

11. Other Notarized Documents (Special Power of Attorney (SPA) / Affidavit of Consent/Invitation/Guarantee/MOA, etc.)
• After the document is notarized, secure Certificate of Authority for a Notarial Act (CANA) signed by the Executive Judge or Vice Executive Judge from the Regional Trial Court which issued the commission of the Notary Public.

12. Immigration Records
• Certified/Authenticated by the Bureau of Immigration (BI)

13. DSWD Clearance
• Travel Clearances for minors issued by the Department of Social Welfare and Development (DSWD)

14. NBI Clearances*
• NBI Clearances for travel abroad must be issued by the National Bureau of Investigation (Green)

Note: Personal copy is not valid.

15. Police Clearances
• Police Clearance issued by the Philippine National Police

Note: Applicant may opt to secure police certification from Camp Crame (Directorate for Intelligence Clearance).

16. Barangay Clearances
• Mayor’s Clearance or Certification which has a jurisdiction over the Barangay of the applicant’s place of residence.

17. Export Documents
• Certified/Authenticated by the Philippine Chamber of Commerce (PCCI), the Department of Health (DOH), Department of Agriculture (DA) or by the Bureau of Food and Drugs (BFAD), depending on the nature of document

18. Business Registration and Other Documents issued by a Government Agency (e.g. SEC, DTI, BIR, Municipal Business Permit & Licensing Office, etc.)
• Certified/Authenticated by the issuing office

 

The DFA San Fernando, Pampanga is open from 10:00 a.m. to 6:00 p.m., Monday to Saturday.
