Passport Application:
- Personal appearance is required for all
- Confirmed online appointment is required except for those entitled to use the courtesy lane.
- Earrings and colored contact lenses are not allowed during data capture.
- Please ensure that information contained in your IDs and Birth Certificates/Marriage Certificates are the same.
- Consular Officials may require additional documents as deemed necessary.
- The public is advised to secure their passports prior to booking their flights. Flight bookings should ONLY be made if passports are still valid for at least six (6) months.
- Applicants are reminded to be 30 minutes earlier on their selected date and time of appointment at their chosen consular office.
- Dealing with fixers is at your own risk and expense.
- All the originals and photocopies of the documentary requirements for passport application shall be prepared.
- DFA adheres to “No Escort” policy; escorts shall be prohibited from entering the DFA consular premises to accompany the applicants.
On Passport Receipt:
- Only immediate family members are allowed to get the passport in behalf of the applicant. Immediate family members include father, mother, brother, sister, spouse, and children of legal age.
- The passport will only be released to an immediate family member with proper authorization letter and photocopy of the IDs of the passport holder and the authorized person.
- The passport of the minor applicant shall be released only to parents or adult siblings or to an authorized representative with Special Power of Attorney or Affidavit of Support and Consent and photocopy of the IDs of the passport holder and the authorized person.
- Passports unclaimed after one (1) year will be cancelled per Department Order No. 2021-012.
- Passport holders thirteen years old and above must sign the third page of the passport.
- Alteration, addition, or destruction of contents is strictly prohibited. Any unauthorized change will render a passport invalid and will have accompanying penalties.
- Using the passport as collateral is against the law and will automatically render the passport cancelled.
- The information provided in the passport application form must be true and correct. The documents that will be submitted should be authentic. Under the law, making false statements in passport application form, furnishing falsified or forged documents in support thereof are punishable by law.
